Your New Patient Paperwork
Please download your new patient paperwork and fill it out as completely as possible before your first appointment. It's important to be as complete as possible with your health history even if you think the conditions are unrelated to what you are seeking care for. ​

Please bring your completed forms along with any imaging or diagnostic studies to your first appointment.
If you can not fill out your new patient paperwork ahead of time, please arrive 15 minutes early to your first appointment.

We strive to make your experience comfortable, relaxing, and pertinent to your health goals. Traditional chiropractic has a stigma attached to it that makes patients feel as if they are a number getting shuffled through the mix with a brief, generic treatment.

We understand that everyone is different and so are your injuries. That's why we take the time to do a thorough consult and examination during your first visit. It helps us develop a specific care plan designed to get you the best possible results.
After your consultation and exam, Dr. Timar will explain what he found, what you should expect from care, and what needs to be done in order for you to recover as quickly as possible.
